Audubon Area Community Care Clinic (AACCC) is a Federally Qualified Health Center funded through Health Resources and Services Administration 330(h) Healthcare for the Homeless grant. AACCC provides health services, referral services and supportive services to uninsured, underinsured, refugee and medically underserved populations within the seven county region of Northwestern Kentucky.


AACCC provides comprehensive medical services including, but not limited to:

· Physical Examinations

· Immunizations (limited)

· Office and laboratory testing

· Care for acute and chronic health problems



AACCC provides quality holistic behavioral health services for its patients that include referrals for:

· Behavioral Health Counseling

· Substance Abuse Counseling

· Dental Care

· Other Health Care Specialties



AACCC provides a full array of services for our patients beyond their immediate health care needs such as:

· Prescription Assistance

· Transportation to appointments

· Vouchers for dental and eye exams

· Case Management

· Application Assister Insurance enrollment

· Interpretation; Translation Services



 Insurance & Payment: Medicare, Medicaid, and most commercial insurances are accepted. Claims are filed for you. We are participating with most insurance plans.


We welcome patients without insurance, and offer a sliding scale of discounts on most services based on household size and income. More information is available from the front desk team. Additionally we have staff in place to help uninsured patients sign up for benefits under the Kentucky Health Benefit Exchange.


Payment of copays, deductibles and co-insurance are expected at check-in. If you are unable to make your payment at check-in, you may have to reschedule your appointment. Cash or checks are accepted. There is a $25 charge for returned checks. Payments are considered past due if all or partial payment is not made within 30 days from the date of the billing statement. A minimum monthly payment of $25 or 1/12 of the balance, whichever is greater, should be the minimum payment on open balances. Payments that are more than 120 days late from the original statement may be turned over to a collection agency. Payment plans are available.
